Are old MOST forms (completed prior to 2015 revision) still valid, or do we have to redo MOST forms for all our patients/clients/residents?
Last Updated: October 4, 2018The older MOST forms remain valid until the form is revoked, the patient expires, or the form is revised at the patient’s request. When revisions or new forms are completed, we strongly encourage facilities to use the more current version of the form.
