PROFILE: Advanced Care Planning Website

The mission of the University Of Colorado Division Of Geriatric Medicine is to be at the forefront of geriatric care. They provide comprehensive patient and family-centered clinical care, conduct cutting-edge research, and train the next generation of geriatricians and interdisciplinary team members. Through partnerships with the Seniors Clinic at University of Colorado Hospital and Rocky Mountain Regional VA Medical Center, they treat older adults who have multiple chronic conditions, striving to help patients and family achieve their quality of life goals, including remaining independent at home. 

 Project Summary:  Launched in July 2018, ColoradoCarePlanning.org is the first of its kind comprehensive website aimed at helping Coloradans of all ages find information for future medical planning. The site provides an overview of advance care planning and guides visitors along a path to think about their values, choose a medical decision maker, write down their wishes, make medical choices, and share wishes with others.

The “What If I” page provides additional advance care planning resources tailored to individuals with different needs – for example, Spanish-speaking individuals, Veterans, and individuals with dementia. Because advance care planning can bring up additional topics such as housing and caregiving, the website also provides Coloradans with local community resources.

Benefit to Colorado:  Serious illness will affect all Coloradans at some point in their lives, either through friends and loved ones or by them getting sick themselves. Having a plan for these difficult situations can make the process easier and ensure that patients’ wishes are honored.
