The Health Equity Fund is accepting applications for new partners and projects for the upcoming fiscal year.

 

Programs and Services

The Health Equity Fund is a partnership between CIVHC and the Colorado Health Foundation (CHF) that's designed to support community-driven work by providing CO APCD data access and program evaluation services to community-based organizations focused on health equity.  Selected projects may receive up to $50,000 to offset costs associated with CIVHC’s services. Please note that this is not a direct grant program to non-profit organizations.

Through the Health Equity Fund, CIVHC’s analysts work alongside community-based organizations to quantify program impact. This analysis can help identify improved health outcomes, reduced emergency care use, health care cost savings, and more. These findings provide powerful insights that demonstrate program value and support grant writing, fundraising, and stakeholder engagement.

Eligibility
  • Community-based non-profits in Colorado with annual operating revenues under $10 million are eligible, though those with larger revenues may also be considered.
  • Projects must focus on health equity.
  •  Organizations that have previously benefitted from the Health Equity Fund are not eligible to apply.
When to Apply

The Health Equity Fund is now accepting applications for projects that will take place during our next fiscal year (July 1, 2026 – June 30, 2027).
