NHDD Advance Care Planning Open House

Trained volunteers from the Conversation Project Boulder County are available throughout the day on April 15th in honor of National Healthcare Decision’s Day to help you with advance care planning questions. They can help get you to the right documents, start an important conversation with your family, and offer questions to consider in the time of COVID-19. Appointments are online or via phone call and last up to one hour. Please note if you prefer a phone appointment. When you sign up, your personal Zoom meeting link will be emailed to you.
