DESCRIPTION:Organizations across the nation break down barriers firmly entrenched in our health care system. With boundless passion and limited resources\, they increase access to care\, implement innovative ways to deliver high quality care at affordable prices\, and\, day in and day out\, fight to keep Americans healthy. \nDuring these chats\, we will highlight one of these Change Agents and the exciting work they are doing to create a better health system for us all. \nChronic Care Collaborative Colorado \nThe Chronic Care Collaborative (CCC) is coalition made up of 34 advocacy organizations representing Coloradans with a range of chronic conditions. The CCC works to improve access to quality\, affordable\, and integrated health care. Member organizations of the CCC each received customized drug dashboards\, populated with CO APCD data specific to the drugs that their particular condition most rely on and utilize. These interactive dashboards show how much Coloradans are spending for certain prescription drugs and whether prices are going up or down for health plans and patients. \nHow are CCC members using the drug information in the reports?
