SUMMARY:Change Agent Chat - Chronic Care Collaborative Colorado
DESCRIPTION:Organizations across the nation break down barriers firmly entrenched in our health care system. With boundless passion and limited resources\, they increase access to care\, implement innovative ways to deliver high quality care at affordable prices\, and\, day in and day out\, fight to keep Americans healthy. \nDuring these chats\, we will highlight one of these Change Agents and the exciting work they are doing to create a better health system for us all. \nChronic Care Collaborative Colorado \nThe Chronic Care Collaborative (CCC) is coalition made up of 34 advocacy organizations representing Coloradans with a range of chronic conditions. The CCC works to improve access to quality\, affordable\, and integrated health care. Member organizations of the CCC each received customized drug dashboards\, populated with CO APCD data specific to the drugs that their particular condition most rely on and utilize. These interactive dashboards show how much Coloradans are spending for certain prescription drugs and whether prices are going up or down for health plans and patients. \nHow are CCC members using the drug information in the reports? What insights can be gleaned from dashboards such as these? \nJoin our 30 minute #ChangeAgentChat on Friday\, December 8th\, 12-12:30pm MT\, hosted by CIVHC’s Chief Medical Officer\, Tamaan Osbourne-Roberts\, MD. SUMMARY:Center to Transform Advanced Care (C-TAC) and CIVHC -- Advanced Illness Care Policy Briefing
DESCRIPTION:The Coalition to Transform Advanced Care (C-TAC)\, the leading voice for people living with advanced illness\, and the Center for Improving Value in Health Care (CIVHC) will host a policy event to brief advocates on the latest policy developments in Washington and around the country that affect this vulnerable population. This discussion will include a national perspective\, a discussion on what is happening in Colorado and in other states\, and how other advocates can support this movement. \nOver 10\,000 Americans become eligible for Medicare every day. By 2030\, the number of Medicare enrollees is expected to double to approximately 80 million individuals – many of whom will have increased healthcare needs later in life. Our healthcare system is currently unable to accommodate the rapidly growing size and need of the aging population. \nC-TAC and CIVHC invite you to a compelling event with national and state leaders to highlight these issues and have a discussion on the role we all can play in supporting people living with advanced illness and their families. You will be briefed on efforts at the federal and local level currently underway as well as engage with a panel of experts who are working to improve health outcomes. Participants include: \nMarian Grant\, Senior Regulatory Advisor\, C-TAC \nBrian Shiozawa\, Regional Director\, U.S. Department of Health and Human Services \nKristin Paulson\, Vice President of Research and Innovation\, CIVHC \nJean Kutner\, Chief Medical Officer\, University of Colorado Hospital \nTracy Lippard\, Clinical Lead for Complex Needs and Elder Care\, Care Management Institute\, Kaiser Permanente \nDebbie Hornor\, Senior Vice President\, Health Strategies\, Southwest Affiliate\, American Heart Association \nAdrian Miller\, Executive Director\, Colorado Council of Churches
