Integrating Public Health to Achieve Triple Aim Goals

In 2011, CIVHC worked extensively with members of its Payment Reform, Delivery System Redesign, and Consumer Engagement Advisory Groups, along with other interested stakeholders, to lay out a framework for moving the payment system toward outcomes-based reimbursement for care in Colorado. The advisory groups developed a six milepost framework that offers a path for communities to evolve to the destination of highly integrated systems of health and health care delivery. The destination includes community and public health as full partners with providers paid risk-adjusted, performance-based global payments, and patients fully informed, engaged, and empowered.

Last spring, CIVHC brought together a small working committee made up of representatives from local public health agencies, the Colorado Department of Public Health and Environment, and commercial insurers and population health experts, to explore opportunities for effective integration of population health with personal health care, better understanding the current landscape, identifying potential gaps or barriers to success, and the potential role for CIVHC in advancing a shared vision of integration.

The group identified that an important initial step would be to gather stakeholders across the health care spectrum to inform stakeholders of the critical role and functions of public health, as well as current challenges, and introduce the group to some innovative approaches to integration underway. Through the group’s work, CIVHC held a Public Health Integration Summit on October 18, 2012. Over 90 participants including representatives from local public health agencies, the Colorado Department of Public Health and Environment, health plans, providers and funders gathered for a full day of learning and thoughtful discussion.
