CORHIO Awarded $1.7M Grant to Improve Coordination for Hospital and Long-Term Care Patients

CORHIO (the Colorado Regional Health Information Organization) has been awarded a $1.7 million grant to improve care for nursing home, assisted living, home health, hospice and hospital patients. CORHIO is among only eight organizations across the country to receive a “State Health Information Exchange Challenge Grant” from the Office of the National Coordinator for Health IT (ONC).

The Challenge Grant allows CORHIO to support long-term and post-acute care providers in Boulder, Colorado Springs, Pueblo and the San Luis Valley as the staffs in these organizations work to implement processes to improve care transitions to and from hospitals in their communities. It also allows CORHIO to offer assistance to long-term and other post-acute care providers in those communities to help reduce the costs for implementing health information exchange (HIE) technology, increasing the ease of adoption for this important segment of health care providers.  This initiative promises to improve patients’ experiences and health outcomes through consistent provider access to patient care summaries and other secure, electronic health information.

CORHIO will work with CIVHC to facilitate a statewide discussion to develop standards, processes, and protocol for improving patient care transitions in communities across the state and the nation.  CIVHC will leverage expertise and experience from a number of Colorado-based organizations in improving care coordination and transitions to define best practices and develop a standard template for adoption by all communities interested in providing better value through their health care services.

Through development of statewide HIE, CORHIO’s work with health care providers in the four targeted communities, and CIVHC’s leadership in facilitating a statewide discussion on best practices for improving care transitions, this initiative represents great potential to realize improved value in patient experience, health outcomes and reduced costs for all Colorado patients transitioning across care settings.
