Our Payment Reform Operations/Implementation Workgroup, comprising members of our previous Payment Reform and Consumer Engagement groups, as well as stakeholders from across the spectrum, is responsible for fleshing out and implementing the Midway Milepost. The Payment Reform Implementation Workgroup performed additional cost and variability analysis on the list of potential bundles proposed in the “Midway Milepost” report. As of November 30, the Workgroup has decided to move ahead with bundles for the following chronic conditions and acute care episodes:
- Coronary artery disease (CAD)
- Coronary artery bypass graft surgery
- Total knee replacement
- Hip replacement

Technical Advisory Group
The Workgroup has tasked a separate Technical Advisory Group (TAG), comprising contracting and coding experts, to flesh out these bundles. The TAG will pursue this work in earnest once the Centers for Medicare and Medicaid Services announces its decision on a bundling “grouper” methodology in January.
As the TAG completes our bundling work, the Implementation Workgroup is now focusing its lens on other elements of the Midway Milepost – specifically, increasing the penetration of patient-centered medical homes and other care coordination payment and delivery models.
